Default One dead rainbow fish/Levels are fine?

This morning I looked in the tank and there was one of the rainbow fish floating at the top dead. I checked the water levels immediatly and found that the ammonia and nitrate were almost non excistent. Ammonia was 0.1 and nitrate was 0

I noticed there was a lot of waste and what not on the bottem so I did a 15 gallon water change and grabbed a lot of the crap on the bottem. I washed a few ornaments and filled it back up?

I am wondering what the cause of this was. I hope it wasn't my feeding schedule? Maybe their was a fight?

Oh yeah and I also notice one single white spot on one of my smaller sucker fish?

Keep an eye on the fish, slowly take the temperature upto 30 celsius..this will not harm the fish and will shorten the life of the ich if it is that. If more spots appear buy treatment.
