Default Tank Problems: Fish dying

Hey all,

About 3 weeks ago, I had a slight ick problem, so I added some salt and raised the temp. and I haven't seen hide or hair since then.

Yesterday, I noticed one of my emperor tetra's, who've I've had in the tank for close to 4 months, was not eating and was puffing hardcore at the surface of the water. I was on the way out the door, but I did water tests before I went out and PH, Nitrate, Nitrite and Ammonia levels were spotless. I went out shopping and came back and he was dead, and then I noticed the same thing on one of my Flying foxes...needless to say, I'm a bit concerned.

I removed the CO2, long ago, because it was causing nothing but PH swings; around the time I did my ick treatment.

What does this sound like? Oxygen problems? Let me know.

I'm not trying to insult your basic knowledge. So please don't take offense to these questions lol.
Do you have an air pump in there?, Are you treating your tap water?(Chlorine and metals). There is something else that may need to be tested. But I can't rember what it is. Have your fish been eating regularly? What have you been feeding them? Did you put the filter cartridge back in? With a filter cartridge you have growth of bacteria on there, and some bacteria is good for fish. What is the temperature of the water? Overheating the water my be what's bothering the fish if you left the heat up too high for too long.

So those are just some questions I'd ask. Maybe it'll help maybe not!

Do you have an air pump in there?

Cannister Filter - Rena Filstar XP2

Are you treating your tap water?
Yes, with hagen water treatment (I hadn't done a water change for 4 days). I add cycle once every week or 2 weeks when I do water changes.

Have your fish been eating regularly?
I feed my fish once a day or less. I use a combination of bloodworms, brineshrimp, and some algae and meat flake.

What is the temperature of the water?
Temp is at 80 - 82 F. Can't remember completely (at work atm)

Did you put the filter cartridge back in? With a filter cartridge you have growth of bacteria on there, and some bacteria is good for fish.
I clean my filter once a week, but I haven't touched it about 6 days. I'm very careful when cleaning my ceramics inthe filter. I clean them with tank water.

Anything else?

Tank size and total inhabitants? Tetras are sensative to ich meds and salt treatments also.

Another Emporer bit it tonight...something is obviously wrong. I'm not completely sure if it is gone. There is no sign of it on the outside of the fish.
